New Patients

Here at CCN we are continuously striving to improve our patient’s satisfaction by providing the highest level of quality and care in Monterey County. We accept most insurances including: Workers Compensation, Medical, Medicare, Private Insurance, HMO, PPO, Tricare, and Triwest. Please call your insurance carrier to see if we are a preferred provider on your plan.

We ask all New Patients to please arrive 15-minutes prior to your scheduled appointment time.  We also ask you to bring the following information:

1. Government issued picture ID (if applicable)
2. Insurance card(s) (Primary and Secondary)
3. Co-payment
4. Payment (If you do not have insurance)
5. Medical Records (If available)
6. All New Patient information filled out (4 Pages)
7. A list of current medications
8. A list of all Drug Allergies
